Jacqueline "Jackie" Polzin, a lifelong resident of Toledo, Ohio, passed away on January 6, 2024, at the age of 82. She worked for State Farm Insurance for over 10 years and was a devoted wife, mother, sister, grandmother, and great-grandmother. She was predeceased by her husband, Carl Polzin; and her parents, Frank and Rose Yunker.
She is survived by her two brothers, Mike and Frank Yunker; her sister, Marie Diehl; her two daughters, Judy Polzin of Toledo, OH and Debra Woodard (Chad) of Jonesboro, AR; her eight grandchildren; and her 17 great-grandchildren.
Jacqueline was in very special hands in Legacy Perrysburg Skilled Nursing and Rehabilitation Perrysburg. To all of the staff and administration at Legacy, words can't describe the gratitude owed to them. As her health declined, she became a patient with Continuum Care Hospice Legacy, where her hospice nurse, Tracey Lacer, was literally her and our Angel who cared for her as more than a caregiver, but also a friend to her and the family in her final months. There isn't enough thanks the family can give to Legacy and Continuum Care for the special treatment and love they showed and poured into Jacqueline's care.
Another very special thanks to her original home aid, Helen Famakinwa, who took her own special time out of her life, to visit, sing, and pray frequently with Jacqueline, while in Hospice care. They grew a close bond and the family is overwhelmed by the show of love and her selflessness of how good of heart she has and the peace she brought to Jacqueline and the family. She passed in peace with a smile on her face knowing so many new faces became so close and loving to her. Thank you all.
The family would also like to thank her neighbor, Charisma Stokes and children for 13+ years of looking after Carl and Jacqueline, helping them through their latter years. Also Victoria McCullough, her most closest granddaughter, who helped tremendously, along with Victoria's husband, Dustin, for going that extra mile for being there with her grandmother and helping making this transition easier.
